Objective:To study the effect of sildenafil (Viagra) on the pharmacokinetics of sildenafil citrate, a potent and selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5) in healthy volunteers. Method: A total of 522 subjects with mild to moderate erectile dysfunction were randomized to receive either sildenafil citrate (5 mg) or placebo for 6 weeks. Sildenafil at a dose of 2 mg/day was given as a bolus. On the following day, the subjects were randomly assigned to receive either a sildenafil citrate or a placebo in either the sildenafil or placebo groups. Blood samples were collected at intervals for 3 to 5 h prior to drug administration, and the area under the plasma concentration-time curve (AUC) was calculated. Plasma concentrations of sildenafil and placebo were analyzed using an validated high-performance liquid chromatography (HPLC) method. The sildenafil and placebo plasma concentrations were compared to the concentrations obtained in healthy volunteers who received either sildenafil or placebo. Mean AUC was calculated as the sum of the plasma AUC and the area under the plasma concentration-time curve for each sildenafil dose. The plasma sildenafil concentration-time profile of sildenafil was similar to that of placebo, with the peak plasma concentration being 1.00 ng/mL in healthy subjects and 0.56 ng/mL in healthy subjects. There was no significant difference between sildenafil and placebo in the AUC of plasma sildenafil concentrations (4.78 ng/mL vs. 0.28 ng/mL; P = 0.9) and in plasma sildenafil concentrations (6.70 ng/mL vs. 2.00 ng/mL; P = 0.1). There was a significant difference in the AUC of sildenafil and placebo (8.30 ng/mL and 8.90 ng/mL, respectively; P = 0.001). In conclusion, sildenafil could be used as a potent and selective inhibitor of the PDE5 enzyme in healthy volunteers. It should be considered that sildenafil has a high efficacy profile and should be used in clinical practice only when it is needed. As the pharmacokinetic properties of sildenafil are not related to any other pharmacodynamic parameter, the use of sildenafil at the recommended doses is justified in patients with the above mentioned clinical findings.
